Activities Participation Requirements
Before students can participate in an activity, parents need to go to "finalforms.com," fill out the required information and submit the required documentation. Here is the pdf outlining this process: FinalForms Directions. Finally, here is the Pre-Participation Physical Evaluation form to be completed for some of our activities.

1. Completion of your FinalForms Online Registration.
2. A current year ASB card.
3. *A current physical on file at HMS. Physicals are good for 2 years and must not expire until the end of the season in which the student is participating.

4. *All activity fees should be paid before participation on TouchBase
