2018 Healthy Youth Survey for 6th and 8th graders A message from HORIZON MIDDLE SCHOOLDear Horizon 6th and 8th Grade Parents,Our district is participating in the Washington State 2018 Healthy Youth Survey during fall 2018. Students at Horizon will be taking the survey on October 11th.The Healthy Youth survey includes questions related to physical activity and diet, unintentional and intentional injury, substance use, risk and protective factors, and access to school-based services. Survey results are used by schools, communities and state agencies to plan programs to support our youth and reduce their risks.Participation in the survey is voluntary and the student's answers are anonymous. Parents or guardians can see a copy of the survey questions in the school office. If you do not want your child to participate, please contact the office to excuse them from taking the test.Please read the attached Parent and Student Notification Letter for more information about the Healthy Youth Survey and feel free to contact the office if you have any specific questions.
